I attest that I am authorized to submit the proxy on behalf of the above-named insured.
This document verifies that the above-named insured is a policyholder of Acuity, A Mutual Insurance Company ("Acuity"); further, it serves to constitute and appoint the CEO of Acuity, the President of Acuity, the Secretary of Acuity or any of them, as my proxy, to vote for me to (1) elect directors at Acuity's annual member meeting and (2) vote on any other matters that may properly come before the members of Acuity at an annual or special meeting of the members, or any adjournment of such meeting. This proxy shall be revoked and expire upon the first to occur of the following: (i) my attending any meeting of members of Acuity and voting in person;(ii) my delivering to the Secretary of Acuity at its principal office either a writing stating that this proxy is revoked or a subsequent proxy appointment; or (iii) five years from the date this proxy is received at Acuity's principal office.
